Academic Planning and Advising

    • Use this form to declare a new major.  You must fill out one form for each major you declare.

    • Changing your mind?  That's ok!  Use this form to drop a major.  

    • Add and drop your minors with this form. 

  • Advising Worksheet

    • This used as an advising tool for some advisors to help them help YOU.

Enrollment and Course Adjustments

    • Use this form to petition to take over 17 credit hours in one semester if your GPA is not established or you do not meet the minimum GPA requirement of 3.0.  

    • Use this form to withdraw from a course after the add/drop period. 

    • This form is used when a student wants to take a course at another institution and transfer it back to 人兽性交 University.

  • Directed Study

    • Under extenuating circumstances, students may elect (with approval) to participate in a directed study during the first 4 weeks of the semester.

Experiential Learning

Program and Graduation Requirements

    • Use this form if your advisor makes a recommendation for a major or minor course substitution.

    • Use this form if your advisor makes a recommendation for a General Education substitution.

  • Waiver of Senior Residency

    • Use this form if you are going to be taking courses at another institution during the last 31 hours of your time at 人兽性交.

  • Petition To Update Catalog

    • Students may opt to update their catalog to a newer version.

FERPA Related

Faculty and Course Management

    • Faculty use this form to override students into their courses for the following reasons:
      1. Their course is full
      2. You are waiving a prerequisite for the student
      3. The Student has a time conflict.

    • Faculty use this form to create curriculum proposals.

    • Faculty use this form to request specific data from the Office of the Registrar or the Office of Institutional Research.

  • Zane State Exchange Petition Form

  • Petition For Traditional UG Student to Take a MAP Course

    • Students that are eligible may petition to take a MAP course.

    • Faculty use this form to make changes to the course schedule.

    • Faculty use this form to change grades in case of a mistake, updated work, or if the student was given an extension.

    • Coaches use this form to add a student into a Varsity PE course credit for a spring or fall semester.  May be filled out anytime during the semester.  The coach is responsible for submitting a grade for this course at the end of the semester.

    • Faculty use this form to change advisors for a student or add a co-advisor. 

Other Forms

  • Lifelong Learning Form
    • Alumni from 人兽性交 University are give the opportunity to take one free traditional undergraduate course per spring or fall semester.  Please see the policy for more details.
  • 人兽性交 Plan/Audit
    • In some instances students want to audit a course tor various reasons. Auditing a course does not count towards academic load and the student does not receive a letter grade.
  • Change of Information (Address/Legal Name Change)
    • Faculty, Staff, and Student use this form to request a name or address change.
  • Transient Student Request
    • Some students that take courses at another institution need to fill out this form in order to prove academic status to the other institution.
